Registrations
We are currently welcoming registrations from patients who are new to the local area. If you are currently already registered at a GP practice in Dundee please check with staff before registering.
When you come to register, you need the following information.
- Registration form
- Additional Information form
- 2 forms of ID. 1x Photo (eg. passport or drivers licence etc) and 1x proof of address (eg. utility bill or lease etc)
- Your repeat medication slip from your previous practice if you are on any repeat medications
Without all the above information, we will not be able to process your registration. Please ensure all forms are fully completed and that you have at least 2 weeks worth of medications from your previous practice. We ask that you hand these forms into the practice between 1-2pm if possible.
Once you return the forms, your registration request will be reviewed before the registration process is complete. We cannot guarantee registrations until the full process has been undertaken. If there are any issues with your registration request - we will contact you. Otherwise after 7 working days you can assume that your registration has been completed.
