New Patient Registration

A new patient registration form is available at the reception desk which must be completed and returned together with a copy of proof of identification for consideration on the practice list. Where patients are requesting to join the practice list, the practice does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition. Once registered, patients may choose whichever doctor they wish to attend in the partnership.

All patients are encouraged to attend a new patient medical as soon as possible after registration. It is a chance to get to know each other and to establish any special health needs you may have.

Non-English Speakers

Pre-registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time to complete the registration process you will require two forms of identification preferably your passport, driving licence and or a utility bill. You will be asked to sign the form to confirm that the details are correct.

Please note you will not be registered with the practice until the forms are completed and proof of identity has been verified.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
