New Patient Registration

registrationA new patient registration form is available at the reception desk which must be completed and returned together with a copy of proof of identification for consideration on the practice list. Where patients are requesting to join the practice list, the practice does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition. Once registered, patients may choose whichever doctor they wish to attend in the partnership.


NEW PATIENT MEDICALS

All patients are encouraged to attend a new patient medical as soon as possible after registration. It is a chance to get to know each other and to establish any special health needs you may have.
